Inflammatory and hormonal influences (oxytocin, prostaglandin, collagenase, and elastase) lead to increased water content and promote the anatomic changes that soften the cervix. Effacement (lengthening of the muscle fibers) and dilation (external os widening) are the physiological changes to the cervix (Anderson & Avery, 2019). An insulin-like ovarian hormone, relaxin, plays a role in maternal hemodynamic by blocking uterine activity, promoting myometrial quiescence, and regulating gap junction (Blackburn, 2013). Collagen bundles are separated in labor and contribute to cervical remodeling.
