Death is the transition phase: it is the end of the known life, but it is also the gate to something new. From our current state of knowledge, it is impossible to know for sure what happens after death, but it is possible to feel it. Thanatology is the science dedicated to studying death: it researches the stages of decomposition of the human body and the near-death experiences (NDE).
Such experiences are helpful to understand what happens after death: those are descriptions of the experience of people who were close to death but survived. I think and feel that humans are transformed after death, according to their deeds during their lifetime. Those who did the good are transformed into a higher state; those who lost the connection with the spirit, doing thebad, are dissolved. I do not know whether we are reincarnated, as I think there is not enough evidence for that.
