A problem is an unwelcome, mostly unpleasant situation that requires a solution to eliminating it. An issue is equally an unwelcome situation but much softer than a problem with a readily recognizable solution. A sub-issue is a part of a more significant issue arising from this more prominent issue and much softer than its source. A root cause, on the other hand, is the initial, real, or only cause of a problem or issue, whether openly seen or subtle. To exemplify the distinction between these terms, consider a cryptosporidiosis outbreak. The cryptosporidiosis outbreak is a problem. The issue in a cryptosporidiosis outbreak is the suffering in terms of stomach aches and vomiting. The sub-issue is the reduced quality of life. The root cause is the presence of cryptosporidium in food or drinking water.
