Discuss diagnosis and management of spontaneous rupture of membranes (ROM) before onset of labor & during labor.

ROM is a medical term implying spontaneous rupture of membranes, and it is a typical element of labor. Commonly, membranes rupture during the labor, but there are cases when it can happen before the start of it, which is called pre-labor ROM. Such a scenario occurs in about 10% of pregnancies, which makes it a commonly diagnosed condition among mothers (Brucker, Jevitt, King & Osborne, 2018). There is another type of prelabour ROM called premature (PPROM), which eventually results from pathology (Brucker et al., 2018). The decision regarding PPROM case is grounded on gestational age, the wellbeing of a fetus, unnecessary complications, and the access to specialized care (Brucker et al., 2018). All these conditions can and must be diagnosed to determine if a mother has to get special treatment. There are currently many ways to diagnose ROM, e.g. by direct observation of amniotic fluid or laboratory tests, including such tests as fern test and nitrazine test. However, a fern test should be seen as a test supporting diagnosis rather than one concluding it. At the stage of clinical management, when the diagnosis is confirmed, medical professionals should decide if the cervix is ripe for labor induction. It is done by either digital or visual examination, based on which a further Bishop score can be given. When a score has been given, a decision for ROM can be taken.
