It appears that hospitals had not initiated an inquiry into the deaths of patients from respiratory or cardiac failure before the Institute for Healthcare Improvement (IHI) initiative because of two reasons. On the one hand, they lacked the insight required to understand the reasons for these adverse events. Nurses were often hesitant to call the code team, and this means that there was low awareness about the issues contributing to patient deaths. On the other hand, even if nurses had complained about the situation, hospitals would have had limited capacity to respond due to the lack of a rapid response team. The processes in the units were highly structured, so it would have been difficult to create additional teams without a proper rapid-response team (RRT) capacity.
