Why do nurses call physicians when patients are in distress, and why do physicians frequently not respond? Why do nursing directors caution nurses against calling the code team?

Nurses’ relationships with physicians are complex and are often guided by scope of practice regulations. These regulations may limit nurses’ ability to provide care to patients in distress because they require physician oversight during certain procedures. Additionally, it is possible that non-ICU nurses are not experienced or trained in the use of equipment required for responding to patient distress. Hence, nurses require other care providers for support, such as physicians or a code team. Attending physicians may fail to respond for various reasons, including being busy with another patient, having a lunch break, or being off-site for other reasons. Calling code teams is also not the best solution in such cases. This is because code teams are required to respond to actively failing patients, and if they are busy with a patient who is showing early signs of distress, they might not be able to save other patients. Hence, code teams might complain about nurses for calling them in non-emergency cases, and it could have a poor effect on nurses’ performance reviews and floor ratings.
