From my perspective, the root of the problems with the organization in general and with the cases considered in particular lies in the significant organizational gaps at the planning stage. Available data show that British Petroleum (BP) suffers from a lack of preparation, expressed in insufficient feedforward control.

The case demonstrates that a company made many mistakes, including constructive ones, to save money and get more benefits. These actions eventually led to the creation of risky situations that were not considered in advance. If the company had taken a closer look at the Thunder Horse oil platform pipes, the risk of their damage could have been avoided.

Pre-planning could also be expressed by creating stricter instructions that the staff had to follow strictly. In this case, the workers at Deepwater Horizon would be forced not to ignore the messages but to report them in time. Finally, BP could have hired more skilled employees focused on safer work rather than profit.
