Suppose the patient experiences issues with osteoarthritis, kyphosis, cataracts, and tinnitus, as well as a fear of falling. Which professionals would best meet the needs of the client/patient in this case? What are the professional roles and responsibilities of each of the professionals?

The interdisciplinary team to work with the patient would thus include a home nurse, a primary care physician, an ophthalmologist, an orthopedist, a neurologist, and a mental health professional.

A primary care physician would be responsible for regular patient exams, referrals to specialists, prescriptions, and the overall coordination of care. They would also play a central role in communicating information to the patient and her family. A home nurse would be tasked with assessing the patient’s living conditions and providing services to enhance the quality of life, such as physical therapy, pain management, and patient or family education. Specialized doctors, including an ophthalmologist, a neurologist, and an orthopedist would play relatively smaller roles in the process. They would be required to assess the patient’s condition based on the physician’s referral, review prescriptions, and conduct any necessary tests or procedures to reduce the risk of falling, improve existing symptoms, and address any other concerns. To foster interprofessional communication, the results should be passed back to the primary care physician. Lastly, the involvement of a mental health specialist is required to help the patient overcome her fear of falling. This team member would be responsible for meeting with the patient on a regular basis and applying age-appropriate strategies to help the patient regain confidence and independence.
