A just culture offers a promising way of addressing and preventing errors in healthcare settings. The model of a just culture involves balancing the core duties of care providers against organizational and individual values. Therefore, in order to ensure that a just culture works as expected, it is necessary for care providers to collaborate, exhibit quality leadership and courage, and trust one another.

Firstly, teamwork and trust help to address errors and prevent them by using the experience and knowledge of individual providers. For example, root cause analysis is an effective way of identifying the causes of medical errors, and it works best when applied by a team. Additionally, these concepts contribute to interprofessional communication and collaboration, which has a positive influence on patient safety. Secondly, leadership in quality improvement is crucial because it aids in improving employees’ motivation, facilitates compliance, and has a positive impact on quality performance. Finally, courage is an important concept in quality improvement because it takes courage to take responsibility and face the consequences of problems. When the concepts of teamwork, leadership, trust, and courage are in force, it becomes possible to develop a just culture.

Unfortunately, my current practice environment does not possess all of these characteristics. Doctors and nurses work together to prevent errors through teamwork and show relatively high levels of trust. However, when a mistake occurs, the management usually blames care providers, even if they did not cause the error. This affects the attitudes of doctors and nurses while also reducing the effectiveness of quality improvement efforts. In order to achieve a just culture, it would be necessary to enhance leadership and courage. These changes would help the management to admit that some mistakes are caused by system factors and implement process improvements correctly, thus fostering a just culture.
