Ethical dilemmas should be handled with logic and facts. Use a decision-making model to evaluate the dilemma of patient autonomy.

The analytical model of decision-making has been successfully used in the clinical environment, and it can be applied to the ethical dilemma regarding the autonomy of patients. This model suggests a framework, according to which the decision-making process is shown as a series of analytical steps. The two main phases of this approach are the identification of the problem and the implementation of the solution. The studied problem is taking a decision about medical intervention when protecting the patient’s health might result in a violation of his or her right to self-determination. Logical reasoning according to rights ethics gives a clear answer for this dilemma and says that medical personnel cannot treat individuals against their will, and the final decision about intervention lay with the patient. However, it is essential to avoid simplistic interpretations of this idea and thinking about medics as technical workers whose goal is to carry out procedures as the patient wishes.

The fact that, unlike patients, nurses are specially trained, informed, and experienced and therefore are capable of making educated decisions puts extra responsibility on them. A nurse needs to choose the appropriate approach to every individual and, knowing that intervention is necessary, inform the person and try to correctly persuade him or her to make the right decision. Research has shown that using best practices can have a significant positive effect and are paramount to get valid informed consent from individuals whose cognition is impaired. Thus, nurses should act respecting the patient’s autonomy, but it is also essential to use an individual approach and appropriate procedure to address each person.
